Insurance
We participate, file, and accept assignment for Medicare, Health First, Aetna, Cigna, United Healthcare, and certain other insurance plans. Please check with our office to see if this applies to you.
You are responsible for any co-payments or deductibles at the time of your visit. Patients are requested to bring their Insurance card and a picture ID on their first visit and when an Insurance change occurs. Please remember that your insurance is an agreement between you and your insurance company. You are responsible for any debt incurred with our practice.
